The Family Impact Assessment asks about some of the hardest parts of a family’s life: money, health, grief, and hope. This page explains plainly what happens to what you share.
What we collect
Only what you enter in the assessment: contact details, information about the athlete and household, the event your family experienced, its financial, medical, educational, emotional, and athletic effects, your stated priorities, and any documents you choose to attach.
Spiritual-care questions are optional. They are used only to route the care you ask for and are never part of any score.
We deliberately do not ask for, and ask you not to submit, Social Security numbers, bank or account numbers, insurance-policy numbers, or full medical records.
Saving your progress
The assessment is long, so your answers are saved as you go. That happens in two places, and they are different.
In your browser. Progress is always saved on the device you are using, so closing the tab does not lose your work. This copy never leaves your device.
On our servers, only once you give us an email address. From the moment you enter an email address, your answers so far are also saved on our servers, so a link can restore them on any device and nothing is lost if this one fails. That copy is kept for 30 days from your last activity and is then deleted. If you never enter an email address, no partial answers are stored on our servers at all.
While an assessment is unfinished, Foundation staff can see that it was started, the email address given, and how far it progressed — so someone can offer help finishing. They cannot read unsubmitted answers. That restriction is enforced by the database itself, not only by the interface: staff accounts hold no permission that would let them retrieve draft answers.
A link to continue is a key. Anyone who has it can open your assessment and see the answers you have entered, so please keep it private. Requesting a new link cancels the previous one.
Who can see a submitted assessment
Once you submit, your assessment becomes a case that authorized Foundation staff review. Access is granted at one of four levels, and each is enforced by the database rather than only by what the screen shows:
- Read only — can read cases and open attachments, and cannot change anything.
- Reviewer — can additionally record case status, assignment, follow-up dates, and internal notes.
- Board member — can additionally record funding decisions and amounts.
- Administrator — can additionally manage scoring settings and who has access.
Every change to a case is recorded with who made it and when. Every download of an attached document is recorded the same way, and download links expire after five minutes.
Your submitted answers are never edited. Staff record their own decisions alongside what you wrote; they cannot alter what you told us.
How the impact score is used
Submitted assessments receive a score across six categories, used to help staff decide which cases to review first. It is a triage tool, not an eligibility or award decision, and every recommendation is reviewed by authorized staff or committee members under the Foundation’s documented policies.
Optional spiritual-care answers are excluded from scoring entirely. Your score is not shown to you, because it describes review order rather than a judgement about your family.
How long we keep it
Unfinished assessments: 30 days from your last activity, then deleted.
Submitted assessments: retained indefinitely. The Foundation keeps a permanent record of the families it has been asked to help, including the decisions made and the reasons for them. If a case is withdrawn from active review it is archived rather than deleted.
Email confirmation codes are kept only as long as needed to confirm an address, and are stored in a form that cannot be read back.
How it is protected
Information is stored encrypted and reachable only over an encrypted connection. Attached documents are held in private storage with no public address; they can be opened only through a short-lived link created for a signed-in, authorized reviewer.
Access controls are enforced at the database level, so an error in the website cannot expose information that the underlying permissions forbid.
Submitting the form does not create an account for you, and you never set a password.
Who else is involved
We use a small number of service providers to run this service: hosting, a managed database and file storage, an email delivery service for confirmations and links, and a bot-protection service on the form. They process this information only to provide those functions.
We do not sell your information, and we do not share it with advertisers or data brokers. We will never send you marketing messages, and we do not share your phone number with vendors or partners.
Your choices
Answer only what you are comfortable sharing. Most questions are optional, and skipping the spiritual-care section has no effect on eligibility.
You can stop at any point. If you have not submitted, you can simply not continue, and any saved progress is deleted after 30 days.

Information about young people
This assessment is about youth athletes, so it is usually completed by a parent or guardian and often contains information about a minor. We collect it only to evaluate support, restrict it to authorized staff, and never use it for advertising or profiling.
Changes to this notice
If our practices change, this page will be updated and the date at the top will change with it.
Pending legal review
This notice accurately describes how the platform handles information, but it has not yet been reviewed by legal counsel. It must be reviewed, and amended where counsel requires, before the assessment is published to families.
